Kidney Problem: Early Biomarkers and Management

Kidney diseases are among the most common and serious health problems affecting companion animals, particularly dogs and cats. Often referred to as “silent killers,” kidney disorders can progress unnoticed until significant damage has already occurred. Understanding early biomarkers, diagnostic approaches, and effective management strategies is crucial for improving outcomes and enhancing the quality of life in pets.

Understanding kidney disease in pets

The kidneys play a vital role in maintaining the body’s internal balance by filtering waste products, regulating electrolytes, controlling blood pressure and producing essential hormones.

Kidney disease can be broadly classified into two types:

acute kidney injury (AKI), which occurs suddenly, and chronic kidney disease (CKD), which develops gradually over time. Chronic kidney disease is especially prevalent in older pets and is irreversible. Early detection is therefore key to slowing disease progression and preventing complications.

Early biomarkers

Traditional diagnosis of kidney disease relied heavily on elevated blood urea nitrogen (BUN) and creatinine levels. However, these markers only rise after significant kidney damage, often after more than 70% of kidney function has been lost. Recent advances have introduced more sensitive early biomarkers: These biomarkers are revolutionizing veterinary nephrology by enabling early intervention.

Symmetric Dimethylarginine (SDMA): A highly reliable biomarker that detects kidney dysfunction much earlier than creatinine. It reflects changes in glomerular filtration rate (GFR) even with mild renal impairment.

Cystatin C: Another promising marker that provides a more accurate estimate of kidney function, particularly in early stages.

Urinary Biomarkers: Proteins such as neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule-1 (KIM-1) help detect tubular injury at a very early stage.

Microalbuminuria: The presence of small amounts of albumin in urine can indicate early renal damage before overt proteinuria develops.

inical Signs and Diagnosis

Kidney disease often presents with subtle and nonspecific signs in its early stages. Pet owners may notice:

  • Increased thirst and urination
  • Loss of appetite
  • Weight loss
  • Lethargy
  • Vomiting or bad breath (uremic odour)

As the disease progresses, signs become more severe, including dehydration, anaemia and neurological disturbances.

Diagnosis

Diagnosis involves a combination of early screening, especially in older pets. It plays a very crucial role.

  • Blood tests (BUN, creatinine, SDMA)
  • Urinalysis (proteinuria, specific gravity)
  • Imaging techniques (ultrasound, radiography)
  • Blood pressure measurement

Management and treatment strategy

While chronic kidney disease cannot be cured, it can be effectively managed to slow progression and improve quality of life.

  1. Dietary Management: Specialized renal diets with controlled protein, phosphorus and sodium levels are the cornerstone of treatment. These diets help reduce kidney workload and minimize toxin buildup.
  2. Fluid Therapy: Maintaining hydration is critical. Subcutaneous or intravenous fluids are often used, especially in advanced cases.
  3. Medications: Proper medicines help manage the disease better.
    • Phosphate binders to control hyperphosphatemia
    • ACE inhibitors or ARBs to manage proteinuria and hypertension
    • Antiemetics to control vomiting
    • Erythropoietin therapy for anaemia
  4. Monitoring and Follow-Up: Regular monitoring of kidney function, electrolyte balance, and blood pressure is essential for adjusting treatment plans.
  5. Emerging Therapies: Advances in regenerative medicine, including stem cell therapy and exosomebased treatments, are showing promise in experimental settings. Additionally, biosensors and point-of-care diagnostics are being developed for rapid and early detection.

Prevention and Pet Owner Awareness

Educating pet owners about early signs, and the importance of routine screening is vital for early intervention.

  • Provide constant access to clean water
  • Avoid nephrotoxic drugs and toxins
  • Maintain regular veterinary check-ups
  • Screen aging pets annually for kidney function
  • Ensure balanced nutrition
